2008 IAM Scholarship Application

This application is used to apply for the IAM.BMEZINE.COM scholarship for the 2008-2009 academic year. One, $1000US undergraduate scholarship will be provided to a member of the IAM community. Applicants do not need to be US residents. College and University terms and performance standards vary consideribly from country to country. If you are uncertain about how any of the standards apply to you, your school, or your country, please post your question in the Scholarship forum or send an IM to wlfdrgn.

Previous winners of the IAM College Scholarship are eligible to reapply. Those who have not won previously will have a 20 point advantage.

To apply for this scholarship, you must submit materials electronically. You may also submit some materials via mail, however all materials must be recieved by the deadline, regardless of the method of submission.

To complete this application, cut and paste this entire application into a word processor of your choice, TYPE in the answers, read and sign the statement at the bottom. If you lose bold, centering, etc, that's ok. See below for requred submission methods. All materials will be acknowledged via IAM message when received.

SCORING SECTIONS

SECTION 4: BME EXPERIENCES
5 points maximum
Each featured experience receives 2 points
Each non-featured experience receives 1 point.
List URL for up to 5 experiences you have submitted. Where possible, list those experiences that list your IAM name.

SECTION 5: BME PHOTOS 5 points maximum
0.1 points for each substantially different photo submitted. Nearly identical pictures will be counted as a single picture.
A maximum of 1.0 points awarded for each BME section (ie: ear piercing, labrets, tribal tattoos, etc) (note: BME Hard/Extreme submissions not counted from minors)
List date of BME update and URL for up to 50 photos you have submitted. Where possible, list those photos with your name on them, or where it is very clearly you based on photos on your IAM page or other identified photos submitted to BME which are tagged with your IAM name. List as follows:

SECTION 6: EXPERIENCE REVIEWS

10 points maximum
This section will be calculated by judges and does not need to be filled in by you.
Information will be verified for all applicants on or around 5/1/2008, and data from that date used.

SECTION 7: BME EVENTS ORGANIZED
5 points maximum for this section.
2 points maximum for events where you assisted.
1 point for every event where you were the primary organizer and a minimum of 20 people attended.
0.5 points for every event where you were the primary organizer and a minimum of 10 people attended.
0.5 points for every event where you provided significant assistance to the primary organizer of the event and at least 20 people attended. Must provide documentation.
0.25 points for every event where you provided significant assistance to the primary organizer of the event and at least 10 people attended. Must provide documentation.
Attendance based on RSVP's.
List URL of IAM EVENTS listing for events, and whether you were primary or secondary organizer.
If you are not listed on event as primary organizer, describe what you did to help with the event.

SECTION 8: REAL WORLD BODY MODIFICATION COMMUNITY INVOLVEMENT
5 points maximum for this section.
List those things which you do in the real-world modification community. This can be anything from working as a piercer or being in a suspension group to designing tattoos for others to piercing gun protests or just about anything else which is related. List involvement here, and include documentation appropriate to your experience.

SECTION 9: REAL WORLD COMMUNITY INVOLVEMENT
5 points maximum for this section.
This is specifically things that are NOT related to body modification. Volunteer activities, team participation, environmental activism, and anything else that's good for the community. List involvement here and include documentation appropriate to your experience.

SECTION 10: LETTERS OF RECOMMENDATION
10 points maximum for this section
Letters are scored as 0, 1, 2, or 3 points each.
3 letters total.
Maximum of 2 letters from IAM members.
Minimum of 1 letter from recent or current employer or instructor.
Letters from community leaders, prominent IAM members, etc preferred. IAM members may send letter of recommendation directly to wlfdrgn via IAM message. All others may mail written letter, on letterhead if appropriate, to submission address at top of application. If the letter is given to you physically, you may forward the letter, or scan and email with your application.

SECTION 11: EDUCATIONAL ACHIEVEMENT
5 points maximum for this section
Provide documentation of high school class rank/percentile, or equivalent and/or GPA.
If you have completed 24 or more credit hours (or equivalent) in college classes, include this information for all colleges you have attended.
If not using a 4-point scale for GPA, explain the grading scale used.
If this is not on your high school transcript, contact your school. They should be able to provide it.

SECTION 12: ESSAY
50 points maximum.
This is the most important part of the application, and should be given the most thought. The question is intentionally left very open. There is no intrinsically right or wrong answer, and no intrinsically right or wrong angle to approach the essay. Approach it in any way which you feel best answers the question. Spelling and grammar do count, but a complete consideration of the topic and clear expression of your thoughts is more important. Research, presented with short, direct quotes (with proper references given) is acceptable, but not required. Examples from personal experience, as well as current and past cultures are encouraged. Minimum 2000 words. No maximum, but remember that longer is not always better. Essay must be submitted electronically, via IAM message to wlfdrgn or email to dfowler@sprynet.com

The following may be submitted electronically, or by mail.

**If less than 24 credit hours, or equivalent, of college classes as of December 31, 2007

  1. High school transcript (including GPA and class rank/percentile) if less than 24 credit hours or equivalent of college credit.

**If not currently attending the college you intend to attend in fall 2008

  1. Copy of letter of acceptance or written statement from you of status of application/decision process.

**All applicants must provide the following

  1. Printed copy of this application, with all answers typed in. Completed application must be signed to be valid. May be scanned and emailed, or physically mailed.
  2. College transcripts (unofficial transcripts ok) for all colleges attended (including GPA).
  3. Verification of application for financial aid. This can be a statement from the school financial aid office, or any other materials, verifying that you have applied for government and school financial aid and did not receive sufficient financial aid to completely cover tuition, books, and fees. This does not include student loans. It must show $1000US minimum in direct costs to you for the year which are not met through other financial aid sources (excluding college loans). Copy of financial aid award letter or bill is acceptable. Contact wlfdrgn for other forms.
  4. Documentation to verify Section 8 experience.
  5. Documentation to verify Section 9 experience.

OTHER DOCUMENTATION

**If claiming points for assisting in an event

  1. The listed primary event organizer must send an IAM message to wlfdrgn verifying that you provided significant assistance in the event, including what you did for the event.

**All applicants must provide the following

  1. Section 10 letters of recommendation. For IAM members, these may be sent via IAM to wlfdrgn. Others may be emailed or physically mailed.
  2. Section 11 class rank/percentile. This must be in writing, on school letterhead, if not included on transcript. Electronic submission is ok.
  3. Electronic submission of essay, either by IAM to wlfdrgn or by email to dfowler@sprynet.com


Terms of application

I agree that scholarship award is conditional upon sufficient college expenses for tuition, books, and fees, and that if I am awarded this scholarship, but do not incur those costs for whatever reason, it will be returned to the fund administrator and be granted to another applicant.
